Welcome to the University of Salford, whether you are a NEW or RETURNING Student, it is very important that you register with a local GP practice, if you are coming to live and study here. Once you are registered we can get hold of your medical history as soon as possible, we can send information to you on relevant health issues, pick up on any ongoing medical problems and sort out any prescriptions you need to request.
We are registering new patients who are students of the University of Salford or staff members. In order to undertake the registration process we require you to complete a Registration Form and also a New Patient Questionnaire. We ask that you complete the online form below; alternatively please complete and hand in one of the paper copies to us Maxwell building.
Acceptance on to the list is as at the discretion of the Doctor and we may ask you for proof of identity. (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take several weeks to arrive at the Practice.
You may be asked to complete additional paperwork such as an out of area form if you live outside of the practice boundary.
We have the right to refuse to register patients if they have a violent flag against their record.
